HomeHousingAutumn Leaf House Autumn Leaf House is a transitional housing program serving seven women who are chemically dependent and their children. These women reside at Autumn Leaf House after exiting inpatient treatment and receive six to nine months of intensive case management, which focuses on life skills, parenting, and self-sufficiency. The women and children are transitioned into the next phase of our supportive housing program (PPW Supportive Housing); this provides an excellent continuum of care. Autumn Leaf House is a program of the Snohomish Family Center.
